Vol.18 N.2 (Browse shelf(Opens below)) Vol.18 N.2 Available Congenital muscular torticollis (CMT) is the most common musculoskeletal disorder in infancy. The aim of this study was to determine the long-term outcomes of different subtypes of CMT using ultrasonographic (US) diagnostic studies, which could affect the length of physical therapy (PT) in infants with CMT and elicit an early referral. We performed a retrospective analysis of 76 CMT infants who were referred for US investigation and PT between January 2014 and May 2017. The PT duration was 149 ± 72 days, and most infants received PT 2 times/week. The depth ratio and difference between the affected and non-affected bilateral sternocleidomastoid muscle (SCM), passive neck rotation, and lateral flexion were significantly improved after PT. The duration of PT and the number of PT sessions were found to be correlated with initial passive neck rotation (p < 0.001, p < 0.001) and lateral flexion (p = 0.031, p = 0.027). According to linear regression analysis, the initial passive neck rotation was the strongest predictive factor of the duration of PT (p = 0.001) and the number of PT sessions (p < 0.001). Muscular torticollis can be effectively treated by traditional PT. A greater limitation in passive neck rotation requires a longer duration of PT, and attention should be paid to this issue during monitoring of the development of infants with postural torticollis. It should be identified as soon as possible, as the condition could be life-threatening. The syndrome is usually diagnosed from a combination of clinical symptoms and signs, chest X-ray, and computed tomography. Ultrasound is widely-used nowadays due to its noninvasive nature and the advantage of ease of access. We report herein a case of SVC syndrome, in which stasis of blood flow within the vein was detected via power Doppler ultrasound, and further appropriate management was performed. Although ultrasound cannot be used to directly detect the SVC owing to the limitations of anatomic structures, it appears that power Doppler ultrasound could be helpful in the diagnosis of SVC syndrome. Many types of food and soft drink can damage our health, not only because they are acidic, but also because they contain a high percentage of sugar, which may have an impact on the oral environment. The majority of orthodontic patients are teenagers; decay and enamel decalcification (white spot lesions) may occur if orthodontic patients drink sweetened beverages and have poor oral hygiene. Previous studies revealed that soft drinks may also cause enamel erosion, corrosion of orthodontic appliances, discoloration of elastic modules and bonding agents, a decreased bonding strength of orthodontic brackets, and so on, which may affect the treatment outcome. This article presents a review of the effects of soft drinks on the oral environment and orthodontic appliances, including elastic rubber bands, springs, metal brackets, wires and aligners, and the risks and implications of soft drink consumption for orthodontic patients. Vol.20 N.4 (Browse shelf(Opens below)) Vol.20 N.4 Available Enteropathy-associated T-cell lymphoma (EATL) is a rare type of gastrointestinal lymphoma that is only seldom reported in Asia, especially in Taiwan. Type II EATL is associated with a high bowel perforation rate. We report a 35-year-old male patient with no previous disease who presented with abdominal fullness and intermittent abdominal pain. He was found to have a bowel perforation and underwent emergency surgery during hospitalization. Subsequent pathology of the resected specimen showed neoplastic cells consistent with type II EATL. This case illustrates the difficulty in diagnosing and treating patients with type II EATL. Therefore, clinicians should be cautious when observing bowel perforation complications in these patients while chemotherapy is being performed. F00044

Vol.20 N.1 (Browse shelf(Opens below)) Vol.20 N.1 Available Ultrasound elastography techniques have been used to measure the stiffness of focal liver lesions including hepatocellular carcinoma (HCC), but the results have been inconsistent. In this study, we investigated the contradictory results by examining the relationships between tumor stiffness and histologic features of HCC. We retrospectively enrolled 51 patients with HCC diagnosed using multiphase contrast-enhanced computed tomography or magnetic resonance imaging and measured the HCC tumor stiffness using the acoustic radiation force impulse (ARFI) technique before surgical resection. Correlations between the stiffness value and histologic findings after surgery were then examined. The mean age of the patients was 66 ± 10 years, and 42 (82%) were men. The mean ARFI value of HCC was 1.98 ± 0.89, and the mean tumor size was 5.3 ± 4.4 cm. There was a weak but statistically significant positive correlation between tumor size and ARFI value (r = 0.288, p = 0.041). No statistically significant relationships between HCC stiffness and clinical parameters (e.g., age, gender, fibrosis stage of background liver, HCC histology grade, and location or angiolymphatic invasion) were observed. After multivariate analysis, the tumor size was the only independent statistically significant factor related to the ARFI value of HCC (p = 0.041). Ceramide is an important cellular membrane sphingolipid that maintains the integrity and function of cells, but excess ceramide impacts the cellular energy metabolism. Fingolimod (FTY720) modulates the sphingosine-1-phosphate receptor and directly interferes with the sphingolipid metabolism by regulating ceramide synthesis in vitro. First, cells were treated in a high-glucose environment with ceramide or fingolimod, and cytoskeleton staining was performed to observe morphological changes. Intracellular oxidative stress was assessed by H_2DCFDA staining. Then, quantitative real-time polymerase chain reaction was performed to measure the mitochondrial DNA copy number. Western blotting was also conducted to evaluate the expressions of mitochondrial markers Tom20 and Tim23, and apoptotic markers Bax and Bcl-2. Finally, mitochondrial respiration was evaluated using a high-resolution respiratory assay. Ceramide treatment led to structural and morphological changes and increased oxidative stress. Additionally, the level of reactive oxygen species (ROS) in the high glucose + ceramide co-treatment group was significantly higher than in the control group. Treatment with FTY720 prevented the decrease in oxidative stress caused by high glucose + ceramide. Notably, mitochondrial biomass decreased in the ceramide-treated groups , observed by measuring mitochondrial DNA and analyzing proteins at Tom20 and Tim23. Furthermore, increased anti-apoptotic levels and restored mitochondrial function were detected after co-treatment with ceramide + FTY720 in a high-glucose environment. In summary, these data indicated that excessive ceramide affects membrane stability through lipid metabolism and reduces mitochondrial function, thereby inducing damage. Treatment with FTY720 inhibited the damage caused by ceramide and promoted mitochondrial function recovery. F00016

However, important behavioral factors associated with drug use in chronic non-cancer pain (CNCP) patients can be neglected in the communication of the PMI. This study aimed to examine the feasibility of the PMI for the evaluation of opioid use in CNCP. This topic was approached through mapping out the association between the PMI and a potential risk factor, attentional bias (AB), for opioid misuse in CNCP patients. Additionally, the association between the PMI and the daily morphine milligram equivalent (MME) was investigated. In this study, 33 participants on long-term opioid therapy in addition to adjuvant medication completed the visual probe task with opioid-related cues as the target. A pharmacist evaluated the PMI by reviewing each participant's medical chart. Correspondence analysis was conducted to reveal associations between variables (PMI against AB, and PMI against MME). Participants who took stronger medication for incompatible pain levels tended to display higher AB for opioids. A strong and positive association was apparent between a high PMI and high AB, while a relatively strong association was found between a high PMI and a low MME. The results of this study could reflect how CNCP patients consider these drugs vital in controlling severe pain. In conclusion, the PMI can be a useful tool to roughly delineate the efficacy of pain management in CNCP patients who are on long-term opioid therapy. However, attention should also be paid to non-cancer pain patients with a high-positive PMI, as it can either indicate the importance of this drug to the patient, or imply a behavioral risk factor for future misuse of strong opioids.
